How big is pharmaceutical misadventure in our communities?

Up to half of all adverse drug events are considered preventable. Current NZ medication error monitoring systems exist to varying degrees at local hospital/DHB level, whilst community level monitoring is absent and at best the only evidence for the occurrence of such events can be identified through indirect sources. Observations of medication error based on reports from adverse reactions reported to the Centre for Adverse Reactions Monitoring suggest that pharmacovigilance systems are able to identify important events that may not be initially perceived as medication error. Incidental monitoring consequential upon reports received through CARM suggest that the considerable numbers of medication error may go unrecognized and therefore reflects missed opportunities for identifying and focusing targeted prevention initiatives.  Enhancement of established national pharmacovigilance systems to incorporate improved medication error surveillance especially due to the close relationship and at times unclear distinction between adverse reactions and medication error is suggested.

Dr Michael Tatley is Director of the New Zealand Pharmacovigilance Centre at the University of Otago’s Dunedin School of Medicine. The NZ Pharmacovigilance Centre operates the Centre for Adverse Reactions Monitoring Programme which receives and assesses voluntary reports of adverse reactions to products including medicines, vaccines and complementary and alternative products used in NZ. Dr Tatley is also principal investigator in the Intensive Vaccine Monitoring Programme designed and implemented to gather in real time community level data to support the Ministry of Health’s surveillance requirements for the ‘tailor made’ Meningococcal B vaccine used in NZ. Dr Tatley is also a member of the Medicines Adverse Reactions Committee, an advisory committee to the Minister of health on safety issues with medicines and vaccines and with colleagues in the Pharmacovigilance Centre provides data and observations on safety patterns and issues arising through national surveillance.
