biography

Gabrielle Gallagher works for the National Heart Foundation as the Cardiovascular Risk Training Coordinator for the ‘Change of Heart’ project. She began her nursing career in the late 1970s. Prior to joining the Heart Foundation, Gabrielle worked as a practice nurse in Mt Roskill for 10 years. In 2004 she gained a Post Grad Dip HSc (Advanced Nursing), and travelled to California in 2005 to observe Kaiser Permanente and Stanford University models of chronic care in primary health. After further university study she joined ProCare Health Ltd in 2006 as a cardiovascular nurse specialist/practice support facilitator. Gabrielle is passionate about the role of practice nurses within the primary care team, in accomplishing a population approach to early identification of chronic disease and appropriate interventions to prevent premature morbidity and mortality.
