Exemplars and solutions for drug safety and efficacy

In 2006 the BPAC polypharmacy program identified a high prevalence of multiple medicines use by people aged over 50.  Further analysis identified factors which may increase drug related problems, including “high-risk” drug combinations, potentially inappropriate drug use, and access to multiple prescribers. Implicit from these observations were other factors which may also increase drug related morbidity, such as lack of centralized medical record, poor communication flow and problems with patient concordance and understanding about their medicines. To illustrate these points we describe three exemplar cases extracted from our analysis and outline some possible solutions to opitimise safe and effective drug use.

Pharmaceutical Programme Manager Dave Woods is a graduate of Manchester University (B.Sc. [Hons]) and the University of Otago (MPharm). Dave has extensive experience in hospital pharmacy, drug information, rational use of drugs and quality assurance. He has published on a range of subjects and holds editorial positions for several international journals.
