Stories from the coal face - practice and reality

It is widely recognised that there is a strong focus on the occurrence and prevention of medication error in hospitals. Additionally there is growing recognition of the challenges facing clinicians as people move in and out of hospital care (Primary-Secondary Interface).

Hospital clinicians are also starting to recognise that increased risk of morbidity due to the unwanted side effects of medicines creates an additional raft of work within the health system.

It is the preventable component of medication adverse events that is the focus of the Safe and Quality Use of Medicines strategy.  Information systems can assist- but they are not the panacea that many people believe- indeed they can cause as many difficulties as they resolve.

This presentation will provide some insight into some of the muddles and opportunities that occur with medication use, with particular emphasis on transfer of care.

biography

Born and raised in Northern Ireland. B. Sc in Pharmacy and M.Sc in Applied Pharmacology , Post-Grad Diploma in Service Management.  Avril currently works for the Waitemata District Health Board.  She has a strong interest in integration between primary and secondary care, and improving health outcomes at population level. Avril is an experienced hospital pharmacy practitioner with a special focus on drug use evaluation (DUE).
